At Heartwood Assisted Living, we strive to provide a positive transition from independent living, to living in a supportive atmosphere. Our home has been designed with the senior in mind. We have mimicked a classic 1960s home with all of the convenience of ADA accommodations. It is important that all of our seniors thrive in our homes. We work hard to ensure that all staff are properly trained and are continuing education.

Small Community

Custom Care Plans
Daily housekeeping activities are performed by staff, so our seniors can rest easy and enjoy their days with private rooms, and staff that is available 24 hours a day. Each resident has a care plan that is tailored to them.

Our residents’ health care needs are our top priority. Because of this, many of our services are built with promoting a healthy and active lifestyle that encourages our residents’ physical and mental health to thrive.

Medication Management

Full Med Support
All residents receive a custom care plan where personal needs are evaluated and monitored. We also do full Coordination with the resident’s personal care team (Physician, hospice, PT, OT, etc)

Activities of Daily Living
An assisted living is a perfect median between living independently but also receiving a little bit of assistance. Seniors will always want their independence because as adults, they deserve it. At Heartwood, we ensure our seniors are able to make decisions for themselves while making sure they know we are there to help.

At Heartwood, it is important to us that our staff is energetic and passionate about all areas of resident care. We want our seniors to flourish in our care and that means taking care of their physical and mental health. Part of how to achieve that is by having our seniors participate in activities like Wii Bowling, Board Games, Movie nights and more.

Dont worry about a Thing
We pride ourselves in being the ultimate “stay-cation” where all vacation services are offered at home. This means three nutritious meals server daily homecooked. Snacks are available at any time of the day. All Housekeeping services (including laundry) and fun daily activities that promote overall wellness.

Our Tiers of Care

Levels of Care Provided by Tier

Tier 1: Independent

Management of Medication which includes reminders of taking medication and ordering / Re-filling prescriptions. This Tier also Includes partial ADL's, meals and clarification of medication in accordance with your care team.

Tier 2: Partial ADL Assistance

This level of care includes all of tier 1 as well as partial assistance in the daily living activities (as needs arise). Things like assistance in getting dressed and shower stand by are a few examples.

Tier 3: Full ADL Assistance

Level 3 care includes all services within Tier 1 and 2, but adds in full support for all activities of daily living. Getting up from bed and/or a chair and assistance with incontinant care (as the need arises) are all included.
